C1 — How to log a safeguarding concern

Section 1 — Intro
Hi, I'm Clive from SafeguardingUK.
In this video I'm going to show you how to log a safeguarding concern on your phone.
There is no training needed — if you can use a phone, you can use this.
Section 2 — Sign in for the first time
Signing in to your dashboard for the first time.
Your manager will have sent you a welcome email.
On that email there is your email address and a temporary password. Remember these for the next screen.
Also on that email there is a button. Click that button to sign in now.
On the next screen there's a login box. Enter your email and the temporary password.
Then click the sign-in button.
The next screen that you see will appear only the first time you sign in to your dashboard.
Enter your new password in both of the boxes on this screen. Then click the Save and Continue button.
That's it. You're into your dashboard. Every time you log in in the future, you will log in directly to this dashboard.
Section 3 — Start a new concern + pick a service user
To start a new concern follow along with these steps.
On your new dashboard, click the Report a Concern button.
On the next screen there are two buttons at the top: Service user concern and General concern.
The Service user concern button is already chosen for you — that button is for someone in your care.
The button for General concern is for anything off-duty.
For this video we are going to use the Service user concern button.
Below the buttons, there is a dropdown to pick the service user — tap the dropdown, or start typing their name.
If you don't see any service users registered to you, get in touch with your manager as they will need to add them.
Section 4 — About the Concern, fields 1–3 (Category + Specific Type + Type)
The next section below is "About the Concern".
There are six fields. These are the first three.
The first field is for the category. Pick the closest that aligns with what your concern is about.
The second field is for the specific type. Once again pick the closest that aligns with what your concern is about.
The third field is for the type. This is about how you know what happened. Choose from:
- suspected
- alleged
- confirmed
- disclosed by the service user
- witnessed directly
Once again choose the one that closest aligns with what your concern is about and what you know.
Section 5 — About the Concern, fields 4–6 (Severity + Location + Date of Incident)
The fourth field is severity — Low, Medium, High, or Critical.
If you're not sure, pick higher. Your manager can lower it later if necessary.
The fifth field is location, where it happened. Choose from the dropdown where it happened.
The sixth field is the date of the incident. Pick the date. If you can't remember exactly, pick the closest.
Section 6 — Description
The next section is the Description section — the most important field.
Tell the story. Who, what, when, where.
Plain words.
Don't worry about grammar or spelling.
Just be as clear as you can.
Section 7 — Immediate action + Submit
The next section is immediate action taken.
Did you do something there and then?
Did you tell your manager? Did you move the resident?
Write it here, keep it short, and again don't worry about the spelling.
There's a photo upload section too — we'll skip that for today.
When you're done, click the Submit button at the bottom of the page.
The concern is now saved with a timestamp.
Your manager gets a notification.
It's in the record permanently.
C2 — Anonymous Speak Up

Section 8 — Anonymous Speak Up + close
One last thing — Anonymous Speak Up.
On this page you will see what your concern is about and describe your concern.
In the first box select from the dropdown the closest that you can find relating to your concern.
In the "Describe your concern" box describe what you have seen, heard, or are worried about. Do not include your own name here. Be as specific as you can: name names, dates, and places where you saw something happen. This will ensure your manager deals with it correctly. If your manager can't identify where it is or who it was about, they can't deal with it.
Use it when you don't want your name on the concern.
